Added Project Type and Project Status columns to the issue report.

Added a column to Task List and Task Resource Report for Related Items - these are similar to attachments but also include those items related to the Task in any affiliation.

Fixes

The receipt is now upload correctly and only the first page of the file is saved with correct filename.
